PROGRAM OVERVIEW

The vision of Declare is β€œTo see a unified church declare God's glory by transforming lives and communities.” The collective Church is an incredible source of hope and healing. We help local churches partner with one another, as well as nonprofit and community leaders to share God's love in tangible ways to bless lives and communities.  Through the partnership of Kettering Health, ADAMHS Board, and Declare we developed a tool to aid in doing just that.

Kettering Health conducted a Community Health Needs Assessment to explore factors contributing to health risks and outcomes, health disparities and available resources. Upon evaluation we identified health concerns and needs within the Miami Valley and developed a plan to aid in making the region β€œA Healthy, Safe, and Thriving Community.” Compelled by these findings, we developed Pathways to Mental Wellbeing.

Pathways to Mental Wellbeing is a resource to help church and small group leaders engage their congregation in topics such as worry, wellness, and mental health. Through our collective efforts, we've assembled a remarkable lineup of experts in mental health, addiction recovery, wellness, and life coaching. Together, we've developed an innovative Vimeo Showcase platform coupled with a dynamic small group discussion opportunity. This content is the culmination of extensive consultations with local leaders, counselors, pastors, and small group facilitators from the Miami Valley. These first videos and discussion guides are just the beginning, aimed at sparking meaningful and healing conversations within our communities.

How to use Pathways to Mental Wellbeing:

  1. Convene your small group

  2. Go to the Pathways to Mental Wellbeing Showcase and choose 5-6 topics to discuss and explore

  3. Download the selected videos and discussion guides

  4. Watch one video each gathering and go over the discussion questions

  5. If there is a referral need, contact the Healthcare Navigator team by calling 937-558-3925 or email at ketteringhealthnavigator@ketteringhealth.org

  6. Encourage participants to launch their own Pathways to Mental Wellbeing small group
